Australian Workforce is looking for a passionate and hardworking Occupational Therapist - Disability Support for a accredited Support Services Provider under the National Disability Insurance Scheme (NDIS), Full time, permanent position for the right candidate in Sunshine VIC 3020 to start immediately. 
REQUIREMENTS/ ROLE & RESPONSIBILITIES:

* Minimum 1-2 years prior experience working in the disability & mental illness support sector

* Some experience working in the NDIS

*Ability to provide functional assessment (to determine patients day to day requirements, strengths & weaknesses)

*Ability to determine what Assistive Technology (equipment and/or modifications) can help patients achieve their goals.

*Ability & experience conducting 'Home Modification Assessments'

* Min 1-2 years experience working as an Independent Support Workers
